Treatment Plans for Various Conditions

Our rehabilitation program encompasses the latest technology and modalities and innovative treatment plans that include individual and group therapy up to seven days a week. We also offer a prosthetic program for amputees, who can be fitted for prosthetics once they are cleared and then get follow-ups and adjustments.

We aim to help each resident achieve the highest level of functioning possible. Whether someone has suffered a fall, had an accident or stroke, or is recovering from surgery, our dedicated staff can help each person overcome specific challenges. Isabella’s short-stay rehabilitation program can address issues related to the following:

  • Joint replacement
  • Spinal cord injury
  • Stroke
  • Neurological conditions
  • Orthopedic conditions
  • Pulmonary conditions
  • Brain injury
  • Amputation
  • Trauma
  • Deconditioning due to illness and hospitalization

After Therapy

Our Care Team will work with residents and their families to develop a discharge plan supporting a comfortable and safe home life. Our rehabilitation Nursing Team will ensure that each resident has the skills to manage daily life beyond the rehabilitation unit.

For example:

  • Walking
  • Assistance with daily living skills, such as meal preparation retraining and tub transfers
  • Communication
  • Use of adaptive equipment
  • Being able to get in and out of an automobile

But our work doesn’t stop there. We also offer education to help each person improve his or her well-being and make healthy lifestyle choices after they return home.
